Office Policies For Our Patients

Thank you for choosing It’s All About The Patient. We understand that you could have chosen any medical center to receive treatment and are pleased you chosen to seek care with us. Our staff strives to exceed expectations in service and in care, so that we can make your experience stress-free and comfortable. In order to make that happen, we have established office polices that will enable us to provide quality care in a timely manner while providing clarity to how our office conducts professional business.

Office Hours:

We are open from 8 am to 6 pm Monday through Friday and Saturday from 9 am to 3 pm. We are closed on Sundays and on all major holidays. Please note that if you call our office during after hours, return calls will be processed the next day or on Monday.

Medication Refills

The routine practice of Soma Care is to write a prescription to cover your medication until your next scheduled appointment. If you keep your scheduled appointments and/or take the prescribe medication as directed, then there will not be a need for additional medication refills prior to your next scheduled appointment. If an exception occurs, please have your pharmacy fax a medication request to (888) 972-1958 at least 7 days before running out of medication. Refills will be considered for patients with a scheduled follow up appointment and on a case by case basis.

Please remember Soma Care to keep their appointment scheduled and to schedule a follow-up appointment before running out of medication. Patients who are prescribed controlled substances will require to be seen by our medical provider(s) more frequently and will be responsible to call the office 7 days prior to completing their current controlled prescription. In addition, you may be asked to perform a urine drug screening test in the office at a $30 cost which you will be financial responsible for to confirm the presence of or absence of controlled substances.

Missed Appointments

We understand that unforeseen circumstances arise and that schedules can be unpredictable. If you need to cancel an appointment or reschedule, please call the office 24 hours prior to your originally scheduled appointment. Patients who do not cancel or reschedule without a 24-hour notice will be assessed a $75.00 fee, which must be satisfied prior to their next scheduled office visit or day of. If you arrive more than 15 minutes late to an appointment this may be considered a no show, and your appointment will be rescheduled and late cancellation fee may be charged. Please note, insurance companies will not pay for missed appointments; payment for missed sessions or late cancellations is the responsibility of the patient.

Insurance

Soma Care accepts most major insurance plans. If you have questions regarding your insurance, please call (480) 550-7460. Please remember it is the patients job to inform our office of any changes in insurance coverage. Failure to comply may result in a delay of claim processing, resulting in denial of insurance payment. Co-pays and deductibles are expected to be satisfied at time of service. If applicable, patients will be billed for services not covered by your insurance plan by our office.

Payments

Soma Care accepts personal checks, cash, Visa, MasterCard, American Express and Discover. Please make checks payable to Soma Care. Returned checks will be assessed a $25.00 fee. Our office makes it a priority to collect all outstanding balances, including payment arrangements made between our patients and the administrative staff. In doing so, accounts that are not on good standing will be outsourced to a third party for the purpose of collection.

Prior Authorization

Insurance companies may require certain procedures and medications to get prior authorization prior to office visitation and medication dispensing. This process can sometimes take a little longer than one would like and may cause you to be frustrated while waiting for approval by your insurance company. Our office will do our best to move these request along quickly, but it is not always possible and may need assistance from you to contact your insurance company directly.

Forms/Letters

We are happy to complete forms/letters for our patients whether they are from or for government agencies, employment, schools or insurance companies. To ensure the completion of these forms/letters, please inform the front office staff prior to your office consultation or at the start of your visit. Depending on the form/letter, there may be a $30.00 fee assessed. Please allow 7-10 business days for all form request to be completed.

Please DO NOT walk into the office and drop forms off to be completed that day or a day of your choosing. We have many patients that all have similar needs and at times these forms can be overwhelming. We ask for your patience and your cooperation throughout this process.

Medical Records

All medical records request must be submitted to our office in writing. To ensure the privacy of this request, please complete a release of medical information and our office will process the request. Patients may request a copy of their medical records one time, free of charge. Additional copies can be requested at $0.80 per page. HIPPA allows medical offices 30 days to process medical records request. Our office makes it a priority to put forth our best efforts to process your requests in a timely manner.
