Patient Rights and Responsibilities
- Every patient will be treated with respect
- Every patient will receive appropriate care and treatment, using individualized treatment plan
- Every patient is expected to actively participate in the development or modification of their treatment plan
- Every patient will be informed of any alternative treatment methods available, if any
- Services will not be denied to patients on the basis of race, color, gender, sexual orientation, national origin, religion, or any kind of disability
- Every patient can refuse proposed treatment, which the patient does not wish to receive
- Every patient will be informed of any risks associated with the treatment to be undertaken
Soma Care reserves the right to terminate the provider-patient relationship under these following conditions:
- When Soma Care The Patient is no longer beneficial to the patient
- When another provider may be a better service
- When payment is not received
- When a patient misses three consecutive appointments without a 24-hour notice or frequent missed appointments
- When a patient does not actively participate in the agreed upon treatment plan
- If a patient’s family or patient becomes aggressive, hostile, or causes any disruption in the workplace
